We take health care reporting quite seriously. There is no perfect way to determine the city’s best physicians but we use a service that we feel is as good as possible. Here are some questions and answers about the methodology. Before you make any decision about physicians based on this list, please be aware of the following.
QUESTIONS AND ANSWERS ABOUT METHODOLOGY
How were the Top Doctors selected? We used the services of Best Doctors Inc. The Boston-based company is the preeminent organization at gathering professional peer ratings. Through the years the company has built a database of professionals who are highly regarded by their peers. Those professionals are in turn interviewed for their recommendations.
What question is asked of the doctors who are interviewed? Best Doctors contacts each doctor on the previous list and asks the same question: “If you or a loved one needed a doctor in your specialty, and you couldn’t treat them yourself, to whom would you refer them?”
Do doctors get a chance to respond to other names recommended? Every doctor has the opportunity both to comment (confidentially) on the other doctors included in his or her specialty and to make additional nominations. As new names are added to the pool, each undergoes the same peer-evaluation process. The company claims to have developed special software to correct for some methodological biases: For example, to detect and correct for suspect voting patterns and to weigh votes according to the ratings of the doctors who are doing the recommending.
What happens to the data? There is a continual refinement of both the voting pool and the nominee pool. Each time a poll is conducted the list is sifted, refined and improved for better representation and more solid consensuses.
How does this differ from local surveys? One major difference: doctors are evaluated by their peers nationwide, not just by doctors in their community. In many areas doctors may be better known and evaluated by those within their specialty groups – regardless of where they live – than by local doctors who may not be as knowledgeable in specific specialty areas.
Do doctors have to pay to be on the list? No! We would never use the list if that were the case. Here is the company’s own statement on that issue: “Best Doctors never takes compensation of any kind from doctors or hospitals in return for listing doctors in its database, nor does Best Doctors pay doctors to participate in its survey process.”
What are some of the rules that the company uses?
• Doctors are allowed to vote on others in their hospital and medical practices. The feeling is that those doctors know their peers best – that’s where the survey gets some of its most outspoken evaluations, good and bad.
• Doctors are never “automatically” re-included. In each biennial poll, current Best Doctors are re-evaluated along with the new nominees.
• All of the voting is strictly confidential.
• Doctors are not notified of their inclusion on the list until after the survey process is completed. Doctors are not required to pay a fee or make a purchase to be included.
How many doctors were surveyed? As part of its nationwide survey, the company interviewed over 30,000 doctors. Surveying was conducted from Jan. 2005 through Sept. This is the most recent, credible survey of doctors.
How about doctors who are no longer practicing locally because of Hurricane Katrina? Best Doctors has issued this statement: “Doctors displaced and/or relocated due to Hurricane Katrina have been updated to the best of our ability. Unless significant changes occur in a doctor’s practice, doctors who relocate retain their Best Doctors status when they change geographical locations. Any doctors we have been unable to locate remain in our database for further research but were removed from the list submitted due to their inaccessibility to your readers. All doctors we were able to locate were sent voting invitations for the Best Doctors 2007-2008 review.”
Are the surveys administered randomly? No. To get opinions with weight and professional credibility, Best Doctors tries to consult the very best. Researchers contact all current physicians on the list – including many department heads at major teaching hospitals – and ask them to rate specialists outside their own facilities. According the Best Doctors, the medical community has been extremely supportive over the past 15 years it has administered the survey, providing a 40 percent response rate.
Whereas the bias? There is no perfect, bias free way to conduct a ranking of any sort. Though Best Doctors has through the years refined its techniques to eliminate biases, any nomination process that relies of peer evaluations will naturally favor more senior doctors who have had time to develop a reputation. Those who are new in their profession or those who have not had much peer interaction will naturally get less recognition. The broadness and the depth of the voting pool help eliminate biases and cronyism that might be reflected in smaller surveys.
How credible is Best Doctors Inc.? We know of no better company for providing this type of information. Best Doctors’ roots begin with the Woodward/White Company of Aiken, S.C. That company was founded by a person who was diagnosed with a terminal illness who discovered the frustrations of facing serious illness but not having any source as a reference for medical help. Woodward/White was once the subject of a favorable report on 60 Minutes. Though Best Doctors is now located in Boston, the research unit is still located in Aiken.
How were the medical categories used in this selection determined? Best Doctors selected them.
How were the five doctors who were profiled in this section selected? They were selected by the editorial staff of New Orleans Magazine. We tried to choose people that represented a variety of specialties.
Is this the definitive list? Of course not, we have any doubt that there are many worthy doctors who were not included in the list. We are confident, however, that all who are listed are truly Top Doctors.
TOP DOCTORS
ANSWERING THE QUESTIONS Dr. William F. von Almen, II, M.D. - Obstetrics/Gynecology It’s not surprising that Dr. William F. von Almen, II, M.D., would be chosen as a top doctor – from a young age, he was driven by knowledge and success. In his sophomore year of high school, von Almen noticed that few students in his biology class could ever answer the teacher’s questions. One of the few to speak up, he became passionate about the biological sciences and determined to become a doctor. “I kept going and it worked,” he says with confidence. He received his Bachelor of Arts in biology from the University of New Orleans in 1976, and continued on to Louisiana State University (LSU) for medical school, graduating in ‘80. Although he intended to specialize in pathology – “The TV show Quincy was very popular at the time,” he says – when he began making rounds as a medical student, he noticed how diverse the field of obstetrics and gynecology (OB/GYN) could be. “You see a lot of pathology [in obstetrics and gynecology], you get to deliver babies and do surgery and medicine. I still enjoy delivering babies, even at 3 a.m.,” he says, smiling. Von Almen completed his OB/GYN internship at Wake Forest University’s Bowman-Gray School of Medicine and continued in his residency at the LSU-Charity Hospital, where he was the OB/GYN Chief Resident of Service in ‘84. He is also a clinical professor of obstetrics and gynecology at LSU.
Now well into his career, von Almen is an experienced surgeon. “Things are changing so rapidly,” he says. “I can remember the ultrasound machines we used to use were as big as a house – then they were only used in research facilities but now every OB/GYN has one.” Von Almen also notes that current genetics research will drastically change his field in the upcoming years. “Seeing what the medical students know and are required to know is amazing, there are things we didn’t even know existed back when I was in school.”
Von Almen has had to be flexible in his career. After working in hospitals, he’s now in private practice. His group, Crescent City Physicians, now operates in the Touro Infirmary. “I still have a lot of say in my practice but there’s somebody behind the scenes running the business,” he explains.
Von Almen certainly knows how to run a practice that’s much more than a business. “For me, it’s very important to interact with the patient on more than a clinical level. I try – as much as possible – to sit down the patient in my office after an exam to talk in a less threatening setting. I feel that it has served me well; you get a different relationship with the patient by doing that. I can find out things they may have been uncomfortable or embarrassed talking about in an exam room.”
These interpersonal skills extend from the exam room to his active and large family. Von Almen praises his wife Madeline, who has “done most of the child-rearing that I’ve missed out on.” The two have five children and their second daughter is currently studying to be a physician’s assistant – in fact, she will be joining her father for a rotation at the end of the year.
With new lives beginning at home and the office, this doctor’s life is not slowing down anytime soon. “I want to work until my kids are out in the world and successful. After that, I’ll just chase them around and see what they do! Spending time with my family is my main goal.”
FIXING A PROBLEM David Beck, M.D. - Colon and Rectal Surgery When Illinois native David Beck, M.D., attended the U.S. Air Force Academy (USAF) in Colorado Springs, he knew he wanted to pursue a major in the biological sciences. Fortunately for his future patients, the only biology major available was pre-med. He completed USAF as a Distinguished Graduate and continued his education at the University of Miami School of Medicine, where he chose colon and rectal surgery as his specialty. “I’m very goal-oriented and I like to find a problem and fix it. I’m also good with my hands,” says Beck of the qualities that make him a successful surgeon in his field. Beck completed his residency in general surgery at Wilford Hall of the USAF Medical Center (WHMC), located at the Lackland Airforce Base in Texas, and his fellowship in colorectal surgery at the Cleveland Clinic Foundation in Ohio. In 1994, Beck took over for Dr. John Ray, who had just retired from the Ochsner Clinic, now Ochsner Health Systems. Pleased to be at one of the Gulf Coast’s largest clinics, Beck currently presides as chairman of his department. He has received numerous military and medical awards, in addition to prestigious faculty appointments and involvement in medical societies, committees and journals. The author of seven textbooks, Beck has published 90 book chapters and monographs, 140 articles and seven video and audio procedural guides.
“Colorectal surgery is a small specialty,” says Beck, who adds that there are only about 1,500 specialists in the U.S. and only about 70 new colorectal surgeons a year. “Being small, it’s a relatively friendly group and there are opportunities to do some new things in this specialty. Plus, it’s a specialty where you can really help a lot of people.” Beck finds the research aspect enjoyable but notes that his job can be challenging. “Ochsner is a referral center, one of the largest in the Gulf South, so we get a lot of complex cases,” he says. However, he has never regretted entering such a challenging field. “All of the colorectal surgeons I’ve met are nice people and relatively happy,” says Beck, because “no one is trying to steal what you do, like cardiothoracics.”
Hurricane Katrina has altered his practice slightly. “We’ve gotten busier since the storm. We didn’t lose any of our staff but the other colorectal practice in town lost two people, so we’ve picked up some of the slack.” He adds that “the storm has brought a different demographic, patients we’ve never seen before who’ve never had access to this kind of care. They’re very appreciative, though.” Beck remains determined – and prepared for any challenge. “I always try to do what is right for the patients.”
Beck and his wife Sharon, a registered nurse, have twin daughters and a son in their early twenties. Mrs. Beck was already a pediatric ICU nurse when they met and Beck says her independence has made it easier for them to coordinate schedules and travel. Beck and his partners are researching new medications and ways to prevent hernias and regenerate sphincter cells, among other projects. As if surgeries and research aren’t keeping this determined doctor busy enough, Beck also plans to help his department expand in the near future. Always working with his hands, the skilled surgeon enjoys gardening, fishing and woodworking in his rare free time.
BELIEVING IN HERSELF Patricia Cook, M.D. - Neurology I hope one day you’ll be one of us,” said a Dominican High School nun to a young Patricia Cook. “Oh no, I’m going to be a doctor,” the freshman replied with confidence. Though she’d never voiced her ambition until that moment, she’d been inspired by a well known female physician who cared for her sister. “She sat with my sister all night long. I was really impressed by that but I never realized how much that affected me until I said I wanted to be a doctor,” says Cook.
Cook mailed her application to Louisiana State University (LSU) School of Medicine in New Orleans without doubting herself for a second. “Not until I put that envelope in the mail had it ever occurred to me that somebody could say ‘no,’” she recalls. They didn’t say “no,” and Cook became one of six women to graduate from LSU School of Medicine in 1960, after which she completed her internship at Southern Baptist Hospital. She originally wanted to pursue internal medicine and even did two residencies in the field, one at Southern Baptist Hospital and one at Tulane Medical School. However, a rotation scheduling conflict at Tulane caused her to consider a different specialty. “Looking back, when I was in my personal library, I realized it was full of neurology books so I must have subconsciously known that it was meant to be,” says Cook. She completed two neurology residencies, at Charity Hospital and at LSU.
Medicine has evolved since Cook’s first days as a neurologist – she emphasizes that these changes can be good and bad. “Patients need to be listened to and nowadays the challenge is that you’re not reimbursed for your time. I still go along as if it were 20 or 30 years ago when it mattered to spend time with the patient,” she says. “The patient will help you make the diagnosis if you really listen to them.” Cook’s husband Ernest is a retired dentist, so with both incomes and no children, money was never an issue. Cook never lets the insurance companies bring her down because she always puts her patients’ needs first. “I enjoy the investigative part of spending time with my patients, I’m like a sleuth! It’s very rewarding,” she says sincerely.
One major positive change to her field is the evolution of numerous medications. “We didn’t have much in the way of medications when I first started,” she says. However, drugs for Parkinson’s, epilepsy, multiple sclerosis and Alzheimer’s disease are now “just exploding ... if one doesn’t work, you finally have another one to try.” Hurricane Katrina made drastic changes to Cook’s professional and personal life. Her husband has been living with Parkinson’s disease and two accidents have left him unable to walk. Cook hired home care workers for her husband and even brought them to Maurice, during the evacuation, however, they left three weeks after the storm and she had to bring him to a nursing home where her sister works. In order to spend time with her husband, she now works a few days a week in Lafayette in addition to seeing patients in her Metairie office. She has no plans to retire and continues to enjoy helping her patients.
An astonishingly active woman, Cook is president of the Figure Skating Club of New Orleans – she even skates herself – and manages to paint, ski and care for her prize-winning Yellow Lab, Dude. She believes balance is a necessity. Cook says, “I’ve always felt you had to have other things in your life besides work.”
TRUMETING SUCCESS Rene Albert Louapre, III, M.D. - Internal Medicine When Rene Albert Louapre, III, M.D., had a severe case of chicken pox as a child, his doctor visited him each afternoon until he recovered. “When I was a little older and deciding whether I wanted to practice law or medicine, that image stayed in my mind. That is what I’ve become,” he says. Louapre attended Loyola University in New Orleans for his undergraduate degree in biology. He then continued to medical school at Louisiana State University (LSU), graduating in 1973. Louapre completed his residency at Charity Hospital in ‘76.
In addition to his family doctor, Louapre had numerous role models to help him solidify his work ethic, bedside manner and people skills. His father was a noted musician who led the Rene Louapre Orchestra. “My father was very well liked in the city, he played at all of the Mardi Gras balls and debutante parties,” says Louapre. The busy doctor inherited the musical gene from his father and played the trumpet through medical school, although he rarely has time these days. “I think my father would be proud of what I’ve become,” he says. Another significant mentor was Dr. Harry Baskin, the former head of infectious disease at LSU. “He was a wonderful clinician. Those were the days before MRIs and CT scans; he would look at the patient and really take time to put the pieces together. I think about him each time I see a complex patient,” says Louapre.
Louapre believes that “putting the pieces together” is what makes his practice so rewarding and he never considered another specialty besides internal medicine. “With internal medicine, you have to look at the whole patient and I enjoy that interaction,” he says. “It’s not just understanding one part of the body, it’s understanding if the patient’s ailments come from stress or even a certain social interaction.” Compassionate interaction with his patients is what makes Louapre truly stand out. Doctors aren’t always able to spend as much time as they should with each patient, but Louapre makes that a priority. “You usually have about seven minutes to examine a patient, but there are ways to make it seem like fifteen minutes,” he explains. “Eye contact and verbal cues are very important. I’m a good listener and I smile at my patients when I listen to them. I’m a caring guy and that’s what patients like.”
Not only does Louapre strive to be a kind physician, he has the business intelligence that can make a significant difference in whether a medical practice is successful or not. After medical school he joined a practice with two other physicians, however Louapre was quickly determined to start his own. “After two years, I rented the place upstairs [from his current office] and eventually hired a partner. Now we’ve expanded into this huge space but it was a major challenge,” he explains. He has carried his interest in the business side of medicine into promoting managed care and electronic medical records. Louapre is active in the managed care group People’s Health Network, acting as a physician advisor, board member and chairman of both the finance committee and credentials committee. He also works closely with Nextgen, an organization that promotes the use of electronic medical records – a technology that Louapre finds extremely helpful in his office.
His practice has grown since Hurricane Katrina, largely due to the fact that they were one of the first groups to reopen (Drs. Louapre, Kokemor and Sarrat, LLC opened in mid–Oct. 2005). Louapre makes rounds at Touro Infirmary and Ochsner Baptist Hospital in addition to seeing his office patients. In his spare time, he enjoys cycling, sailing, cooking and enjoying fine New Orleans restaurants. His three grown children are successful in their chosen fields – none of which includes medicine! “They saw with me that the lifestyle was too stressful – too many long hours!”
An inspiration to those in any field, Louapre says, “The success I’ve had is directly related to the fact that I love what I do.”
MEETING OF THE MINDS Marilyn M. Skinner, M.D. - Psychiatry Marilyn M. Skinner, M.D. always knew she wanted to practice psychiatry. A native Texan, she graduated from Southwest Texas State University in 1969 and completed two years of medical school at the University of Texas Medical Branch in Galveston before transferring to the Tulane University School of Medicine, where she completed her degree in ‘74. When she found herself on psychiatry rotation at Tulane, however, the work wasn’t exactly what she had in mind. Her patients had serious mental illnesses and needed medication, not therapy. “I thought psychiatry was about having this person lying on the couch, talking about their hopes and dreams. I wanted to help people put their lives back together. So in my residency, I was just flabbergasted,” says Skinner. The altruistic young resident became depressed and asked her professors for a reference for a psychiatrist of her own. As it turns out, her professors recommended a psychoanalyst. “This is what I want to do!” she exclaimed to her doctor upon realizing that she was lying on a couch, on the opposite side of her dream profession. After her residency at Tulane, she enrolled in the New Orleans Psychoanalytic Institute and is now one of the most successful psychoanalysts in the city.
“The human mind is the neatest thing in the world,” says Skinner. She is inspired by the opportunity to listen to peoples dreams, fantasies, relationships and fears in order to them find their own path. “My relationships with my patients can be very intense,” she says. Skinner believes one of the biggest challenges of this field is transference and counter-transference between doctor and patient. “The feelings your patient has about you and the feelings you have about your patient can be difficult to overcome,” Skinner explains. “It’s not always ‘I love you,’ from the patient sometimes it’s ‘I hate your guts.”
She remains professional yet compassionate toward her patients and feels that they have the answer to their problems. “Yes, I can prescribe medicine but he or she needs to recognize that my job is to help them uncover their own answers,” she says. Some patients complain that she doesn’t talk enough or provide concrete answers; she explains, “I’m not a tour guide; I don’t know what you ought to do. I might have an opinion but frankly I’d be offended if someone told me how to run my life.”
Skinner’s practice has changed drastically since Hurricane Katrina. Many of her patients have left the city or state – Skinner was able to have phone sessions with many patients while living in Birmingham – and she has new clients that need a different level of care than her typical patients. “I’m dealing with more medication than ever before,” she says. Many new patients are paying through insurance, which is a major contrast to her usual private-pay patients. She emphasizes that all of her patients are experiencing a high level of stress and anxiety due to post–Katrina issues such as crime, rebuilding and lack of adequate government support. Skinner believes, “things are getting better but not at the rate they should be.”
A natural teacher, Skinner is not only an associate professor at LSU and Tulane medical schools, she’s the chair of the New Orleans Psychoanalytic Education Committee. She has served as president of both the New Orleans Area Psychiatric Association (1980-‘81) and the American Medical Women’s Association (‘79-‘80) in addition to holding numerous other professional leadership roles.
Her husband passed away from cancer a year before the hurricane, however Skinner is happy to have her son Robert and two cats to which she is “fanatically and neurotically devoted.” In addition to reading, golf lessons and the occasional bridge game, Skinner has re-entered the dating world. “It’s scary out there,” she laughs. “I try not to analyze them!”
